Xanthogranulomatous pyelonephritis (XGP) is a rare chronic kidney infection that can cause progressive decline in function and lead to removal or death. Symptoms of right flank pain due XGP, sometimes radiating the lower abdomen were difficult assess distinguish from urinary tract diseases alone accompanied by involvement other organs like appendicitis. We report fifty-year-old woman was referred because suspected mass based on ultrasound, but results Ct-Scan with contrast showed stones parenchymal tendency pyelonephritis. There no post-contrast enhancing masses both kidneys. Then, decided perform nephroureterectomy found inflammation her appendices. Histopathology xanthogranulomatous The patient diagnosed Chronic CT-Scan, previous ultrasound appendicitis, there kidney. During nephroureterectomy, appendicitis Pyelonephritis after histopathology examination. This describes uncommon confirms difficulty differentiating XGP even though it occurred large city. Moreover, be lesson for general practitioners more thorough diagnosing abdominal pain.
